條款：

  1. 香港醫護市場調查及策劃有限公司（下稱「本司」)為是次活動的統籌單位。
  2. 閣下參加此活動之同時，即代表同意此等條款及細則，並同意成為本司會員及接受本司擁有此等條款及以下所述的權利。
  3. 每人只可參加活動一次，重複遞交參加者將不獲受理其參加資格。
  4. 閣下必須符合資格，本司將有可能要求出示附合此計劃的資格證明。
  5. 閣下報名時必須按指示填妥參加問卷及繳交資料，否則當棄權論，額滿即止。
  6. 本司有權暫停、修訂或更改此等條款及有關是次活動的各項安排，並可隨時取消或終止活動，事先毋須另行通知，亦不需要為此承擔任何責任。
  7. 本活動捐贈之產品及提供服務由贊助商送出(如有)，本司只負責義務安排。本司不會就產品負任何責任。
  8. 閣下如參加此計劃及領取產品後需按建議使用產品。
  9. 參加者明白及同意活動屬自願參與性質，參加者願意承擔一切風險及責任，並無權向主辦機構／包括大會、活動策劃及贊助商／支援機構追討由往返活動場地中、活動中發生或其他原因而引致任何形式的損失索償或追究責任。
  10. 每位參加者必須確保身體健康狀況是適合參加是次活動。主辦機構／活動策劃在懷疑的情況下，保留取消任何不適宜參加者參加的權利，而相關人士必須接受主辦機構／活動策劃的決定。
  11. 參加者必須確保在報名表格上之資料正確無誤。
  12. 若參加者在活動期間因任何行為、過失或蓄意破壞而導致主辦機構／活動策劃支付額外開支，必須對主辦機構／活動策劃作出合理之賠償。
  13. 閣下同意分享予 本司之圖/文/片段/錄音將有可能予本司及活動/計劃之合作商/贊助/協辦方作日後推廣之用，而本司所贈予的產品/現金券/服務/金錢視為報酬，不能撤回。如 閣下不同意請不要提供。
  14. 如有任何爭議，本司保留最終決定權。
  15. 此免責條款的中英兩個版本有任何抵觸或不相符之處，亦以中文版本為準。

私隱政策聲明:本司非常重視閣下的私隱。閣下向本司提供信息及個人資料純屬自願，倘若閣下不希望閣下的資料被用作下文所述的用途，請不要向本司提供有關資料。

閣下提供的資料有機會作下列用途：

本司聯絡閣下領取證書、產品、計劃必須的跟進行動或為閣下傳送閣下有可能感興趣之資訊、優惠、進行市場調查等。

如欲取閱及/或對閣下向本司提供的個人資料作出修改或倘若閣下希望我們不再使用個人資料與閣下聯絡，請電郵與我們的職員聯絡，而閣下會因此而放棄有關參與本司活動、優惠及領取證書等資格。

Terms and Conditions:

  1. Hong Kong Healthcare Research and Consulting Co., Ltd. (hereinafter referred to as “the Company”) is the coordinating unit for this event.
  2. By participating in this event, you agree to these terms and conditions, agree to become a member of the Company, and accept the Company’s rights as described in these terms and below.
  3. Each person can only participate in the event once, and repeated submissions will not be accepted for participation.
  4. You must meet the qualifications, and the Company may require proof of qualifications that comply with this program.
  5. When you register, you must follow the instructions to complete the registration form and submit the information, otherwise, it will be considered as a waiver, and the event will end when it is full.
  6. The Company reserves the right to suspend, revise or change these terms and any arrangements related to this event, and may cancel or terminate the event at any time without prior notice, and does not need to assume any responsibility for this.
  7. The products donated and services provided in this event are sent out by the sponsors (if any), and the Company is only responsible for arranging them. The Company will not bear any responsibility for the products.
  8. If you participate in this program and receive products, you need to use the products as recommended.
  9. Participants understand and agree that participation in the event is voluntary in nature. Participants are willing to assume all risks and responsibilities, and have no right to claim or pursue responsibility from the organizing body/including the conference, event planning and sponsors/support organizations for any form of loss caused by traveling to and from the event venue, during the event, or for other reasons.
  10. Each participant must ensure that their health condition is suitable for participating in this event. The organizing body/event planner reserves the right to cancel the participation of any unsuitable participants in case of doubt, and the relevant individuals must accept the decision of the organizing body/event planner.
  11. Participants must ensure that the information on the registration form is correct.
  12. If a participant causes the organizing body/event planner to pay additional expenses due to any behavior, negligence, or intentional damage during the event, they must make reasonable compensation to the organizing body/event planner.
  13. The data collected in this event, shared images/text/clips may be used for analysis and promotion by our company and partners/product parties. If you do not agree, please do not provide.
  14. In case of any dispute, the Company reserves the final decision.
  15. In the event of any conflict or inconsistency between the Chinese and English versions of this disclaimer, the Chinese version shall prevail.

Privacy Policy Statement:

The Company highly values your privacy. The information and personal data you provide to the Company is purely voluntary. If you do not wish your data to be used for the purposes described below, please do not provide the relevant information to the Company.

The information you provide may be used for the following purposes:

The Company may contact you to collect certificates, products, necessary follow-up actions for the plan, or to send you information, offers, conduct market research, etc. that you may be interested in.

If you wish to access and/or make changes to the personal data you have provided to the Company, or if you wish us to stop using your personal data to contact you, please email our staff. As a result, you will forfeit the qualifications to participate in the Company’s activities, offers, and collect certificates, etc.
